Lower Full Arch Dental Implant Rehabilitation

Our patient came to us needing a full lower arch restoration. Discover how fixed implant-supported treatment helped restore function, aesthetics, and long-term oral health.

Lower Full Arch Dental Implant Rehabilitation

A comprehensive lower full-arch rehabilitation case demonstrating a fixed dental implant-supported reconstruction to restore function, aesthetics, and oral health.

What was happening

  1. Failing lower dentition with over-erupted incisors and a deep bite, causing excessive loading on the upper arch
  2. Lower arch gum disease and insufficient posterior support
  3. Class II skeletal relationship

Initial plan: reset and rehabilitate the lower arch; the upper arch was staged for later treatment

Treatment steps (lower arch)

Extraction of compromised lower teeth and placement of an immediate denture to assess adaptation to a new lower incisor position and bite.

Patient outcome: satisfactory aesthetics and bite, but retention was a concern; patient requested a fixed solution

Four dental implants placed evenly along the lower arch, guided by the provisional denture tooth position

Before Treatment

Immediate post-op plan: allow osseointegration for ~4 months

Soft-tissue assessment after osseointegration

Finding: insufficient soft-tissue thickness and a minimal band of keratinised tissue

Intervention: free gingival graft taken from the thick palatal tissue to augment peri-implant gingiva

Outcome: significantly improved gingival quality around final implant abutments

Definitive prosthetic reconstruction

Scans used to design a fixed lower bridge with a metal substructure and acrylic-wrapped teeth

Result: correctly positioned, fixed teeth that also corrected the anterior overbite for proper function. Bridge was made cleansible for easy hygiene maintenance beneath the bridge and around the implant abutments.
